By registering a minor/participant for a LENAH class, session, workshop, or event, I confirm that I am the parent or legal guardian of the participant and that I have the authority to enroll the participant and to agree to the terms below on the participant's behalf.

I understand that LENAH classes, sessions, workshops, and events involve physical movement, mindset work, and breathwork, including yoga-style poses, strength and stretching exercises, guided breathing, and related activities. I give my permission for the participant to take part in all of these.

I understand that LENAH is responsible for the participant only during the scheduled class time. Instructors supervise the participant during that one-hour class block only. Before class begins and after class ends, LENAH is not responsible for the participant, including any time the participant is at the studio outside of the scheduled class. I am responsible for the participant's supervision, drop-off, and pick-up outside of the class hour.

I understand that physical activity carries inherent risks, including the risk of muscle strains, sprains, falls, or other injury. I have considered these risks and choose to enroll the participant voluntarily.

I confirm that the participant is in good enough health to participate. I will inform LENAH in writing of any medical condition, injury, or limitation that could affect participation, and I understand it is my responsibility to keep that information current.

To the fullest extent permitted by law, I release and agree not to hold liable LENAH LLC, its owner, instructors, contractors, host venues, property owners, and anyone acting on its behalf, from any claims, demands, or causes of action for injury, loss, or damage arising out of the participant's involvement in LENAH classes, sessions, workshops, or events, except where caused by gross negligence or intentional misconduct.

I hereby grant LENAH LLC permission to take and use photographs and/or video recordings of the participant during classes and events for promotional, marketing, and educational purposes (including website and social media use). I understand that no compensation will be provided for the use of these images. If I wish to opt-out of this media release, I agree to notify LENAH LLC in writing at hello@lenahcollective.com prior to the first class.

If the participant is injured and I cannot be reached, I authorize LENAH's instructors to seek emergency medical care for the participant. I understand I am responsible for any resulting medical costs.

I understand that LENAH is a movement and mindset program, not a medical, psychological, or therapeutic service. LENAH and its instructors do not diagnose, treat, cure, or prevent any physical, medical, or mental health condition, and nothing offered in class is a substitute for care from a licensed physician, therapist, or other qualified professional. If the participant has a medical or mental health concern, I am responsible for seeking appropriate professional care on the participant's behalf.

Refunds can be requested before the season begins. To drop your enrollment, notify us by email at hello@lenahcollective.com. At that point your spot will be offered to the next participant on the waitlist, and you will receive a full refund. Once the season starts, we are unable to offer refunds. We appreciate your understanding. If your participant is unable to continue due to injury or illness, email us and we will handle it on a case-by-case basis.

At checkout, typing your full name as your signature and checking the agreement box confirms that you are the parent or legal guardian of the participant, that you have read and understand this Liability Waiver and the Refund & Cancellation Policy, and that you agree to them freely. Whatever name you type at checkout will be accepted as your signature.
